Edition 2: Letter of Medical Necessity:

The letter of medical necessity is an official document justifying the medical need for a product or service you are seeking for your child.  Most funders' require you to have someone substantiate why your child needs this device or service.  They must have a therapist or doctor show why this equipment will improve your childs' health or quality of life.  Below are some simple things you need to make sure the clinician includes in the letter.
  1. Describe who you are (clinician), what you want, and beneficiary’s name (childs)
  2. Establish your credentials (clinician), experience in the field and relationship to beneficiary (child)
  3. Explain the beneficiary’s (childs) condition, including diagnosis, or nature of injury
  4. Discuss the impact on the beneficiary’s (childs) and caregiver’s (parents) life. Note both the limitations and abilities without the requested equipment
  5. State the type of equipment and accessories being requested
  6. Describe equipment, adjustments for growth, and psychological benefits
  7. Describe why the device is medically necessary. Show how the requested
    equipment will result in an increase of function and other physical benefits
  8. Make the person real 
  9. Reiterate and summarize the cost benefits
    1. Note: If the equipment you are requesting is more expensive then some other equipment in that category make sure you explain why the more expensive equipment is needed.
  10. Don’t forget to include pictures of the desired equipment with your child in it (if possible).  This also relates to make the person real from #8.
  11. Signature of clinician
